FINDINGS OF FACT

{¶ 1} On May 21, 2009, plaintiff, Jeremy Chavis, an inmate formerly incarcerated at defendant's Ross Correctional Institution (RCI), was transferred from RCI to the Southern Ohio Correctional Facility (SOCF).
