{¶ 1} Plaintiff, Mumin Israfil, an inmate formerly incarcerated at defendant's Warren Correctional Institution ("WCI"), alleges that on September 25, 2009, he was "denied access to medical care for treatment at a pain management clinic appointment" in that defendant refused to transport plaintiff via wheelchair. Plaintiff asserts that there is documentation in his medical file establishing his need for wheelchair assistance...
