In February 1999, claimant sought medical treatment for wrist and elbow pain that was related to her work. After her employer filed a C-2 form in February 2000, the employer's workers' compensation carrier accepted the claim and approved payments for medical treatment. Claimant treated with an orthopedist throughout 2000 and was diagnosed with bilateral carpal tunnel syndrome. She returned to the orthopedist in June 2003 and authorization was given by...
