Claimant, a kitchen worker, sustained a left leg injury in September 1989 for which accident, notice and causality were subsequently established. In November 2001, claimant was awarded a schedule loss of use of 7.5% based upon a May 2000 C-4 form and accompanying narrative submitted by Dwiref Mehta, her treating physician.
