{1} Defendant Harla Webb signed the written consent form to allow a piercing to occur on a minor without obtaining authorization or permission from the minor's parent or legal guardian. Though the piercing was successful, the minor sustained serious injuries as a result of an accidental fall in the tattoo parlor.
