Statement of the Case

J.P. ("Mother") appeals the trial court's adjudication of her five children as Children in Need of Services ("CHINS"). Mother raises two issues for our review, but we address a single dispositive issue, namely, whether the trial court erred when it found her children to be CHINS. We reverse.
