Several nursing homes submitted annual reports disclosing their income and expenses to the Iowa Department of Human Services (DHS). The reports were used by DHS to calculate the Medicaid per diem reimbursement rates for the nursing homes. Some of the nursing homes' expenses were disallowed by DHS, which adjusted those reports and reduced reimbursement rates accordingly.
