This case comes to us for a second time. Lisa Pakovich worked as a retail sales representative for Verizon when she became disabled and sought long-term disability benefits under Verizon's ERISA plan ("the Plan") after a series of back surgeries. On the first appeal, we reversed the district court's decision that Pakovich was ineligible for long-term disability benefits after the first twenty-four months and remanded to Broadspire Services, Inc...
