(collectively, "Plaintiffs"), commenced this action for damages and equitable relief, alleging that United Healthcare of the Midwest, Inc. ("United"), wrongfully denied or reduced 295 health care insurance claims. United removed the action because 289 of those claims were submitted under employee welfare benefit plans...
