The primary issue in this case is the validity of an "Uninsured/Underinsured Motorist Bodily Injury Coverage Form" (hereinafter "UM selection form") that was initialed and signed in blank by a representative of the insured, and subsequently completed and backdated by an insurance agency employee who then sent the form to the insurer.
