Stevie Johnson ("Plaintiff'), a custodial maintenance worker for the. City of Winston-Salem ("Defendant"), developed bilateral carpal tunnel syndrome, gout, and arthritis, and claimed disability benefits resulting therefrom. Defendant denied Plaintiffs claim by filing a Form 61 with the Industrial Commission.
