Movant, David W. Williams, KBA Member No. 77103, petitions this Court, to enter an order permanently disbarring him from the practice of law in the Commonwealth of Kentucky. Box 557, Simpsonville, Kentucky 40067, requests permanent disbarment for his numerous charged and uncharged violations of the Kentucky Rules of Professional Conduct as follows:
