MEMORANDUM OPINION

In this case, the Court is asked to determine whether the "year-to-date" statement of income given by an employer to an employee constitutes "other evidence of payment received within 60 days before the date of the filing of the [bankruptcy] petition" for purposes of § 521 of the Bankruptcy Code.
