On the evening of January 25, 2001, claimant, a heavy smoker with a family history of heart disease, was admitted into the hospital with a "presumed diagnosis of unstable angina" and myocardial infarction following complaints of on and off chest pain over the previous four days. Indeed, testing confirmed that he had suffered from a myocardial infarction. Claimant, who performed maintenance and construction work for his employer, never returned to work...
