In December 2004, petitioner, a prison inmate, was charged with refusing a direct order and refusing to comply with urinalysis testing procedures. Following a tier III disciplinary hearing, he was found guilty of both charges. In January 2005, petitioner was charged with violating the same prison disciplinary rules in connection with a separate incident and was found guilty following a tier III disciplinary hearing...
