In 2004, claimant, Alton Snearl, was injured in the course and scope of his employment with Kelly's Industrial Services, Inc. ("Kelly") when a "bush hog" tractor fell on him and crushed his ankle. Immediately after the accident, claimant was taken by ambulance to the emergency room of Baton Rouge General Medical Center. Dr. Ronald D.
