Rockwood Casualty Insurance Company ("Rockwood") issued a workers' compensation policy to the Carousel Hotel ("Carousel") in October of 1997. Thereafter, Carousel failed to pay the premiums, prompting Rockwood to mail a Notice of Cancellation ("notice") to Carousel on December 20, 1997.
