Plaintiff alleges that she sustained a work-related injury on 26 October 1998. Shortly thereafter, defendants filed a Form 19, recording that a co-worker grabbed plaintiff by the arm, and spun her around, causing pain. Defendants paid plaintiff's medical bills from 26 October to 10 November 1998 while the claim was being investigated. Some time later that fall, plaintiff filed a Form 33 requesting
