On February 20, 2001, Ronald Lewis ("Lewis") filed a civil action against Horace Mann Insurance Company ("Horace Mann"), among others, in the Cuyahoga County Court of Common Pleas. After the Court of Common Pleas dismissed one of the defendants for lack of personal jurisdiction, Lewis voluntarily dismissed Horace Mann and the other remaining defendants.
