(Rodmar Enterprises), and its insurer, American Interstate Insurance Company (American Interstate), dismissing Mr. Ray's claim for workers' compensation benefits, ordering forfeiture of Mr. Ray's right to benefits pursuant to La.R.S. 23:1208, and ordering restitution in the amount of $117,149.54 for amounts paid by American Interstate.
