Plaintiff Judi Baker ("plaintiff") suffered knee, arm, shoulder and neck injuries when she slipped at and fell while at work. Plaintiff's employer, Sam's Club ("defendant"), paid disability compensation and medical treatment costs related to the knee injury, but denied compensability for the arm, shoulder and neck problems.
