Phyllis J. Mitchell's ("Phyllis") and Jack H. Mitchell's ("Jack") marriage was dissolved in Clinton Circuit Court. Pursuant to the terms of their Dissolution Settlement Agreement, a $100,000 life insurance policy on Phyllis's life was to be gifted to their daughter, Jessica.
