This is a civil action for monetary relief in which plaintiffs Jim and Barbra Klassy allege that defendants Physicians Plus Insurance Company and its medical director, Dr. Gary Johnson, violated federal and state law by refusing to pay for Barbra's bloodless hip revision surgery.
