On June 16, 2000, the plaintiff, Milton Head, filed a disputed claim for compensation against the defendant, Winn-Dixie, Inc., alleging that he sustained injuries to his lower back, including a herniated disc, while in the course and scope of his employment as a forklift operator at the Winn-Dixie Distribution Center in Hammond, Louisiana. Winn-Dixie filed an answer in which they denied that the plaintiff had an accident within the course...
