On January 6, 1995, Grace Green was injured when she slipped and fell on an icy sidewalk at a PNC Bank in Richmond, Kentucky. At three different times on the morning of the incident, a bank teller had spread a melting agent on the sidewalk. However, at the time Green fell, which was approximately 1:30 p.m., the sidewalk had not been treated for over 1½ hours.
