Employer Northwest Shake Tile, Inc., petitions for review of an order of the Workers' Compensation Board that affirmed two occupational safety citations and associated penalties relating to employer's roofing business.1 Respondent Oregon Occupational Safety and Health Division (OR-OSHA) issued those citations for: (1) failing to afford adequate fall protection in violation of 29 C.F.R. § 1926.501(b)(11)
