GOSHORN, J.

Appellant appeals the final judgment terminating his parental rights to his daughter, C.V. Appellant raises several issues, one of which we find dispositive. We reverse and remand for further proceedings consistent herewith.

Appellant and his ex-wife ("Appellee") divorced in 1993. In the dissolution judgment, Appellee was awarded primary custody of C.V., and Appellant was allowed visitation. In March 1995, Appellant's visitation rights were suspended...
