A physician in a workers' compensation case submits a medical-legal report. Labor Code section 4628, subdivision (b)1 requires, among other things, that the physician performing the evaluation sign the report and disclose the name and qualifications of each person who performed services such as diagnostic studies "in connection with" the report.
