Bobbie Duchek appeals the district court's1 grant of summary judgment dismissing as time-barred his claim for medical benefits under a health insurance policy issued and administered by Blue Cross and Blue Shield of Nebraska and Health and Management Strategies, International (collectively, "Blue Cross"). We affirm.

Duchek received inpatient hospital care for alcoholism from September 27 to October 24, 1991...
