Sidney A. Head, Sr. ("Ward") is an incapacitated adult, suffering dementia caused by Alzheimer's disease, who lived in Georgia until Alan L. Head, his son, under his durable health care power of attorney, moved the Ward from a nursing home in Georgia to a nursing home in Arizona.
