On April 15, 1996, defendant, Johnny E. Clark, was charged by bill of information with one count of D.W.I., fourth offense, a violation of La.R.S. 14:98(E). On May 13, 1996, defendant entered a written plea of not guilty in which he also requested a jury trial.
