BACKGROUND

The Debtor is a tenant in a Chicago Housing Authority ("CHA") development. In January, 1996, the CHA served a 14-day Notice of Termination upon the Debtor for failure to pay rent. The notice contained the statutorily required language, including a statement of the amount due, and provided (emphasis added):
