On 28 January 1991 plaintiff sustained a back injury while working as a "rewinder helper" for defendant Federal Paper Board Company, Inc. Plaintiff was out of work until 6 February 1991. The parties entered into a Form 21 Agreement, which compensated plaintiff for a five percent permanent partial disability of the back.
