Plaintiff Samuel Richardson was injured on September 8, 1992 when a bus, in which he was a passenger, stopped abruptly propelling him to the floor. Plaintiff informed the bus driver who gave him her badge number. Two months after the accident, the plaintiff sent defendant Manhattan and Bronx Surface Transit Operating Authority an application for no-fault benefits. This application provided the date, time and place of the accident and a brief description of how it occurred...
