Theresa L. Guidry had an on-the-job accident about July 1, 1989 while she was working for Doctors' Hospital of Opelousas as a Licensed Practical Nurse (LPN). She was paid workers' compensation weekly benefits of $106.15, together with all medical expenses, until November 14, 1991, when benefits were terminated.
