OPINION

SEERDEN, Justice.

Pursuant to a plea bargain, appellant was convicted of forgery, sentenced to five years in prison, and fined $300. We affirm.

In his first point of error, appellant contends the evidence is insufficient to support his guilty plea. The evidence which the State introduced to support appellant's guilty plea was a preprinted form titled "Defendant's Waivers and Judicial Confession." The form contained blank spaces to be filled...
