Eva Lyde purchased two life insurance policies from United Insurance Company of America through its agent, Al Chaney. One policy was purchased in 1979 and insured the life of her husband, Johnny Lyde (Lyde policy). The other policy was purchased in 1983 and insured the life of her son, Glen Smith (Smith policy).
