Claimant Robert Lawlyes filed an application for adjustment of claim, pursuant to the Workers' Compensation Act (Act) (Ill. Rev.Stat.1985, ch. 48, par. 138.1 et seq.). Therein he alleged injuries to his right arm and hip arising out of and in the course of his employment with Huber Brothers (the employer). The case was heard by the arbitrator on September 12, 1990.
