On November 11, 1986, a semi-trailer truck rear-ended and side-swiped a car driven by Janice Scivally. The accident aggravated pre-existing lower back problems experienced by Ms. Scivally. Ms. Scivally applied for disability insurance benefits under Title II of the Social Security Act, 42 U.S.C. § 401 et seq.
