Plaintiff, Irene Schons, as Executor of the Estate of Kenneth Schons, brought an action in equity for reformation of an insurance contract issued by defendant, Monarch Insurance Co. of Ohio (Monarch) and procured through defendant Crump Aviation Underwriters, Inc. (Crump Aviation). Plaintiff's action for reformation of the contract was predicated upon her claim that the contract had been executed under a mutual mistake...
