Atlantic Foundation and Executive Risk Consultants are the employer and carrier (e/c), respectively. Treatment of Gurlacz by Gregory Lovaas, M.D., was authorized by the e/c and Dr. Lovaas submitted bills to the e/c for his treatment of the claimant.
