In this direct appeal, appellant Bonnie K. Hourihan challenges her plea of guilty. For the reasons stated below, we reverse.

I. FACTS

Appellant Hourihan was charged in a three-count indictment relating to marijuana-growing activities. The following notation, apparently referring to count two, appeared on the first page of the indictment at the top right hand corner under the case number: "[21 USC § 841(a)(1) — 5 to 40 years...
