The marriage of Barbara and Allyn Anderson was dissolved by a decree of dissolution on November 14, 1985. Among the provisions in the decree was the following:

IT IS FURTHER ORDERED, ADJUDGED AND DECREED that the Respondent (Allyn) be and he is hereby ordered to obtain and maintain major medical and dental insurance for the benefit of the minor children of the parties and provide appropriate claim forms and proof of insurance to the Petitioner...
