FACTS

On October 24, 1988, Jane Meyer commenced an action against Employers Health Insurance Company ("Employers") and Fleet Services, Inc., in the Circuit Court for Milwaukee County, Wisconsin, alleging that defendants arbitrarily and capriciously refused to pay for medical treatment covered under her insurance contract.1
