Appellant Blackwell appeals from a decision of the Industrial Commission denying his claim for worker's compensation benefits. The Industrial Commission, after conducting a hearing, found that (1) Blackwell failed to prove that he incurred an industrial accident and injury as alleged in his application for hearing; and (2) Blackwell did not give notice of injury to the employer within sixty days as required...
