Defendant, Oscar Paul Berthelot, was charged with three counts of forgery in that he falsely made a United States Military DD-214 form (count I); an academic transcript from the University of Southern Mississippi (count II) and a State of Louisiana pre-employment application (count III), and knowing them to be forged he issued them to the Belle Chase State School with the intent to defraud.
