Appellant, Theresa Quam filed suit against Louisiana Hospital Service, Inc. (Blue Cross) seeking payment of medical and hospital benefits amounting to $4,449.54, together with penalties and attorney's fees. The aforementioned amount was incurred in October of 1985 when appellant had a hysterectomy after learning that she had a carcinoma of the uterus.
