The self-insured employer disagrees with the deputy commissioner's retroactive authorization of treatment by claimant's chiropractor, and with his inclusion of unreported tips in claimant's average weekly wage. Claimant contests the deputy commissioner's failure to assess penalties against the employer for late payment of temporary total disability benefits, and with his failure to compute the compensation rate using claimant's recomputed average...
