Plaintiff's complaint seeks a declaratory judgment that Florida Statute 382.16(5)(c) is unconstitutional. She also seeks injunctive relief requiring the Defendant to issue a birth certificate which he has refused to issue in the form requested by the Plaintiff because of the challenged statute.
