Per Curiam.

This is an appeal from an order of the Court of Common Pleas striking the notice of appeal filed by the employer, General Motors Corporation, as being insufficient to comply with R. C. 4123.519. We reverse.

R. C. 4123.519, in its relevant part, provides:

"Notice of appeal shall state the names of the claimant and the employer, the number of the claim, the date of the decision appealed...
